Critical Care Paramedic (CCP)

The scope of practice for Critical Care Paramedics (CCP) in Nova Scotia is regulated by the Nova Scotia Regulator of Paramedicine (NSRoP). CCPs must maintain their certification through ongoing professional development, continuing education, and periodic recertification.

Critical Care Paramedics (CCPs) have the highest level of training and scope of practice among paramedics. They are equipped to manage the most complex and critically ill or injured patients, often in specialized transport situations such as air medical services. Their scope of practice includes advanced medical procedures, extensive patient care responsibilities, and the use of sophisticated medical equipment. Here’s an overview of their scope of practice:

Advanced airway management

  • Endotracheal intubation
  • Performing cricothyrotomy in emergency situations
  • Utilizing mechanical ventilators and advanced methods of providing respiratory support

Cardiac Care

  • Advanced Cardiac Life Support (ACLS), including manual defibrillation, synchronized cardioversion, and transcutaneous pacing
  • 12-Lead ECG Interpretation
  • Using a broad spectrum of medications including antiarrhythmics, vasopressors, and inotropes

Critical Care Monitoring and Management

  • Using invasive and non-invasive techniques to monitor and manage blood pressure, cardiac output, and other hemodynamic parameters
  • Continuous monitoring of end-tidal CO2 levels to assess ventilation status
  • Assessing and managing patients with severe neurological conditions, including the use of intracranial pressure monitoring

Intravenous and Medication Administration

  • Establishing and managing IV and IO lines for fluid and medication administration in critically ill patients
  • Administration of medications including sedatives, paralytics, analgesics, vasoactive drugs, thrombolytics, and antibiotics
  • Administering transfusions of blood and blood products when necessary

Specialized Procedures

  • Performing needle thoracostomy and tube thoracostomy for patients with pneumothorax
  • Comprehensive management of severe trauma cases, including advanced hemorrhage control techniques and complex fracture management
  • Managing complex wounds and applying advanced dressings

Patient Transport and Care

  • Providing advanced care during the transport of critically ill patients, including in air medical (helicopter and fixed-wing) environments
  • Managing the transfer of patients between medical facilities, ensuring continuity of care for critically ill patients

Step 1
Become a Certified Advanced Care Paramedic (ACP)

Successfully complete a recognized Advanced Care Paramedic program, and obtain certification and registration with the Nova Scotia Regulator of Paramedicine (NSRoP).

Step 2
Gain work experience as an ACP

Accumulate experience working as an ACP. Most CCP programs require a minimum amount of field experience (often 2-3 years) to ensure you have a solid foundation of advanced practical knowledge and skills. Participate in continuing education and professional development opportunities to enhance your knowledge and skills while working as an ACP.

Step 3
Complete a Critical Care Paramedic program

Enrol in and complete an accredited Critical Care Paramedic program. These programs take several months to a year to complete. The CCP program will cover advanced topics such as advanced airway management, mechanical ventilation, hemodynamic monitoring, advanced pharmacology, critical care transport, and advanced trauma and cardiac care. Participate in advanced clinical placements and field internships to gain hands-on experience with critical care procedures under the supervision of experienced CCPs and healthcare professionals.

Step 4
Obtain certification as a Critical Care Paramedic

After completing the CCP program, pass the provincial certification exam administered by the College of Paramedics of Nova Scotia (CPNS). This exam assesses your advanced knowledge and critical care skills.

Step 5
Register as a Advanced Care Paramedic

Apply for registration as a CCP with Nova Scotia Regulator of Paramedicine (NSRoP). Provide proof of your CCP education and certification. Be prepared to meet continuing competency requirements, which include ongoing education and professional development to maintain your CCP registration.

Step 6
Maintain certification and registration

Engage in continuous professional development to keep your critical care skills and knowledge up to date. This may involve attending specialized training courses, workshops, and conferences. Periodically, you will need to recertify your CCP credentials. This may involve passing recertification exams and demonstrating continued competence in advanced practice.
